Minors must be accompanied by a parent or guardian. If accompanied by another adult, they should bring written permission giving authorization to treat in the presence of a responsible individual.
Please cancel 24 hours in advance to avoid any fees.

Appointment Cancellation Policy
Scheduled appointments which are cancelled with less than 24 hours notification or missed without a call maybe subject to a fee, $25 (in office), $15 (telemedicine), and $50 (any physical/procedure). New patients may need to place a deposit if with previous no show or frequent cancellations. The cancelation and no show fees are the sole responsibility of the patient and must be paid in full before the next appointment.
